Yoga Vista Academy doesn’t provide disclaimer/consent forms. If you have insurance coverage for teaching fitness classes, most insurance carriers have templates of what they require should any incident occur.

Alternatively, you may want to ask the location where the interview and class will take place. Some locations already have signed forms from their participants and may provide coverage for you.

If you don’t have insurance and the location doesn’t provide coverage, I suggest you look into getting insurance especially if you’re planning on teaching live or online classes.
